A Chronic Disease Clinical Network is being established to support health professionals working with people with chronic conditions and to empower their greater involvement in the planning and evaluation of chronic disease programs and services. What will the Network do? The purpose of the Chronic Disease Clinical Network is to facilitate collective effort to change how chronic disease is managed in Tasmania and move towards a health and wellbeing system that makes it easier for people to stay healthy, identifies disease early and eases the burden for those who are living with chronic conditions. The Chronic Disease Clinical Network will also be the key body responsible for the implementation of Connecting Care: A Chronic Disease Action Framework for Tasmania 2009 – 2013. How will the Network work? The Chronic Disease Clinical Network will be lead by a respected, inspiring and committed clinician. There may be more than one clinical leader to represent the various professions, disciplines, services and regions dealing with chronic disease across Tasmania. The Clinical Leader will be supported by a broad Steering Committee, the Tasmanian Clinical Advisory Council, the Office of the Chief Health Officer and System Development within the DHHS Care Reform Unit. How do I get involved? Anyone with a role or interest in the prevention, detection or management of chronic disease is encouraged to join - clinicians, private services, consumer groups, community organisations, NGOs, researchers, educators, policy officers and so on.
