RPA0001 is an application for a new licence to deal with X-ray apparatus.  The application can accommodate the licence holder details and one person and one X-ray unit.  To include more people or X-ray units with the application for licence use the RPA0001 ‘Additional’ forms as needed.

Change in contact details

RPA0010 Change is to be used when notifying there has been a change in contact details, business (trading name only), change in partners or partnership or a change in Directors for company licences.  The Radiation Protection Act 2005 does not provide for licences to be transferred therefore in the case of a company or business being sold the new owner will need to apply for a new licence.

Transport radioactive material

RPA0015 is an application for a new licence to transport radioactive material.  The application can accommodate the licence holder details and one person and all transport categories for radioactive material. To include more people with the application for licence use the RPA0015 Additional Persons form.

Mutual Recognition

Applying for a licence when you already hold an equivalent licence in another Australian jurisdiction or New Zealand.  Download the appropriate licence application form and submit it with a completed statutory declaration form.

The place in which you wish to carry out your radiation practice must also comply with relevant radiation safety standards and, to show this, the place must have a certificate of compliance issued by an accredited person.

When a place has been issued with a certificate of compliance by an accredited person an application to register the place may be submitted using the RPA0101 application form.
