Eastern Shore Community Nursing provides comprehensive nursing care, assessment, referral and clinical management to clients of all ages in their home. For more information, see our brochure.

Programs

Community Nursing Clinics

This service allows clients to book appointments for the treatment required at a suitable time to fit in with their lifestyles

Located at: 16-22 Bayfield Street, Rosny.

Referrals can be made by anyone:

  • Individuals
  • Family
  • GPs
  • Carers
  • Health care providers

The individual’s consent is required

Eastern Shore Leg Club

  • Thursdays 10:00 am - 2:00 pm
  • Eastern Shore Leg Club provides community based treatment, health education and ongoing support for people with leg ulcers, with an emphasis on social interaction
  • No appointments required for advice and members can drop in to chat over a cuppa.

Pathways to Change

  • A program focusing on enhancing self management for people with chronic conditions, through face to face and telephone health coaching with the support of a self management worker. Practical skills for people with ongoing health conditions.  For more information phone 6282 0350.

Ambulatory Care Centre

Specialising in vascular access

  • Intravenous infusions
  • Intravenous medication administration
  • Central Venous Access devices - PICCs, Infusaports and Hickmans
  • Venesections for hemochromatosis

Smoking Cessation

  • Mondays and Wednesdays by appointment
  • Face to face education and support for those wishing to give up smoking

Hours of Operation

Home visits

Monday - Sunday

  • Day Visits:  8.00 am - 4.30 pm
  • Evening Visits:  5.00 pm - 9.00 pm

Clinic Appointments

Monday - Friday 8:30 am - 4:30 pm

Weekend appointments are available by arrangement.

Normal office hours are between 8.00 am and 4.30 pm Monday to Friday.  After hours and when the office is unattended calls with be directed to an answering machine.

All contact to community health nurses is through nursing reception.

What to expect from Community Nursing

On admission to Eastern Shore Community Nursing, one of the Registered Nurses will conduct an initial assessment, and plan care with the client and/or family. During this first visit, the nurse will also deliver care if required. The assessment may take longer than subsequent visits and may be completed over a number of visits.

Discharge

Clients will be discharges from Eastern Shore Community Nursing when:

  • Their goals are met
  • They no longer require our services
  • They are admitted to hospital or residential care
  • They move out of the area

Please also be aware that services may be limited or withdrawn if either the client's or staff's safety in compromised.
