Services Available
Our nursing service includes assessing and coordinating individual’s health care needs in a client centred approach to facilitate:
- hospital avoidance
- early discharge and clinical care
- chronic conditions self management
This includes:
- Complex medication management – including support and administration of medication via various routes and devices
- Simple and complex wound management
- Continence advice and support with:
- indwelling and suprapubic urinary catheter care and changes
- stoma care
- associated referrals
- holistic, inclusive palliative care service in association with specialist consultants. Including:
- symptom management
- pain control
- end of life care and planning
- bereavement and grief support
- referrals

Group and individual programs to enhance self management for people with chronic conditions

- Monitoring/Telehealth

- Provision of care coordination/case management and referral pathways
Leg Club
“Leg Clubs aim to provide leg ulcer management in a social environment, where members are treated collectively and the emphasis is on social interaction, participation, empathy and peer support where positive health beliefs are promoted.“

Ambulatory Care Centre
Specialising in vascular access
- Intravenous infusions
- Intravenous medication administration
- Central Venous Access devices - PICCs, Infusaports and Hickmans
- Venesections for hemochromatosis
Collaboration
Working together with the client, the GP and other service providers to develop a holistic approach to care, to achieve the best possible outcomes.

Fees
Pensioners:
- $5.00 - 1 visit per week
$10.00 - 2 or more visits per week
Other:
- $20.00 - 1 visit per week
- $30.00 - 2 or more visits per week
A waiver of fees may be considered in certain circumstances. Further info may be obtained from the Community Health Nurse.
