Huon and Kingborough Community Nursing

Our nursing service can assess and coordinate care for you, a member of your family or a person you care for.  Care will be timely and responsive to the person’s needs.

Referral to this service can increase support to you in your home, may avoid hospital admission and can promote earlier discharge from hospital.

The community nursing service is not an emergency service. If you are experiencing a medical emergency, please telephone 000.

The services we provide include:

  • assessment and planning of your health care needs
  • supporting you with your medications
  • dressing your wounds
  • advice and support if you have a catheter or stoma
  • assistance with treatments for chemotherapy clients
  • antibiotics delivered by injection or a drip.

Our nursing team provide support for end of life care, which involves:

  • managing your symptoms
  • managing your pain
  • end of life care planning
  • grief support and referral.

Our nursing team can provide you with support and education if you suffer from a chronic condition such as:

  • diabetes and associated complications
  • respiratory and cardiac diseases
  • muscular and skeletal conditions.

Our professional nursing service can help you:

  • promote your health
  • create a plan of care
  • maintain independence and well-being at  home.

We can refer you to other members of our health care team:

  • social workers
  • speech pathologists
  • occupational therapists
  • podiatrists
  • physiotherapists
  • respite services and carer support
  • volunteer organisations
  • hospice support
  • Aged Care Assessment Team
  • continence nurse
  • palliative care nurse.

Referrals can be made by anyone, including yourself, your family, your doctor or a health professional.

  • Telephone My Aged Care on 1800 200 422 if you:
    • are 65 years of age or older or
    • are 50 years of age or older and identify as Aboriginal or Torres Strait Islander.
