The aim of the Diabetes Centre is to:

Facilitate self care, promote and maintain health and as a result prevent or minimise the risks associated with diabetes.

The key objectives of the Centre are:

  • Education
    Clients will be able to make informed lifestyle choices to limit the development and/or impact of complications associated with diabetes whilst maintaining quality of life;
  • Social Support
    Appropriate support networks will be established to facilitate self-care, promote and maintain health and prevent or minimise the risks associated with diabetes;
  • Treatment
    Clients will receive acute health care to resolve and/or manage presenting problems and optimise best health outcomes;
  • Resourcing
    Clients will have equitable access to appropriate resources to assist each individual to manage their diabetes in his/her own environment;
  • Research
    Clients will benefit from innovative, evidence-based practice; and
  • Professional Development
    Clients will benefit from quality services delivered by a qualified and competent multi-disciplinary diabetes team.

The Diabetes Centre offers the services of a multidisciplinary diabetes team including:

  • Endocrinologist / Physician / Paediatricians;
  • Diabetes Educators;
  • Dietitian;
  • Social Worker; and
  • Podiatrist.

These services are available as group educational seminars and individual appointments.

Referral

Referral to the Diabetes Centre is not required if you seek education, support or resources from a Diabetes Education, Dietitian or Social Worker. You will however need an appointment. A referral from your treating doctor is required when seeking an appointment with a Diabetes Medical Specialist or the Podiatrist.

Hours of Operation

Appointments Only - 8.30am to 5.00pm Monday to Friday (except Public Holidays)

Reception Only - 8.30am to 3.00pm, Monday to Friday (except Public Holidays)

Ambulatory Care Program - Operates between 3.00pm and 5.00pm Monday to Friday (except Public Holidays). This service offers ambulatory insulin stabilisation and can be contacted by phone, fax and email.

After Hours Emergency Contact

This service is provided to assist individuals manage DIABETES EMERGENCIES such as sick days, hypoglycaemia and insulin stabilisation at home.

The service operates between 5.00pm and 10.00pm Monday to Friday and 8.00am to 10.00pm Saturday, Sunday and Public Holidays.

Location

The Diabetes Centre is located on Level 3 of the Launceston General Hospital. Facilities include comfortable offices and consulting rooms, a training room and meeting facilities. The Centre is wheelchair accessible and has disabled toilets. A range of interpreter services can be engaged on request, and as indicated.
